PURPOSE: The short foot exercise is a rehabilitation exercise that emphasizes activation of the intrinsic foot musculature. This exercise is used by clinicians in rehabilitation settings to treat patients with a variety of lower extremity alignment and function problems. There is very limited evidence to suggest it is effective. The purpose of this study was to evaluate the effects of the short foot exercise on lower extremity function and alignment in physical therapy patients being treated for lower extremity injuries. METHODS: Eighteen subjects (10 males and 8 females, average age 31Б10.3 years, average height 172Б8.8 cm, average mass of 77Б12.1 kg) volunteered for the study. All subjects were patients in physical therapy and were prescribed the short foot exercise as a part of their treatment for a lower extremity injury. Measurements of alignment and function were taken at baseline, two-week and four-week follow ups. Measurements included leg length, standing Q-angle, standing rearfoot angle, navicular drop, evaluation on the step-down test, intrinsic foot muscle test and completion of the Lower Extremity Functional Scale (LEFS). Subjects were taught the short foot exercise after baseline measurements and asked to complete 100 repetitions of the short foot exercise every day during the course of the study. Dependent variables were leg length (cm), standing Q-angle (degrees), standing rearfoot angle (degrees), navicular drop (mm), score on the step down test (6-point scale), a grade (poor, fair, good) on the intrinsic foot muscle test and a score on the LEFS questionnaire. A 1x3 repeated measures ANOVA was performed with time at three levels (baseline, two-week, four-week), the alpha-level was set a-priori at .05. RESULTS: Analysis of the within-subjects effects indicated a statistically significant difference in the step-down test score over the four week study period (P<.001). A follow-up of pairwise comparisons indicated a significant difference between baseline (4.8Б1.1) and two-weeks (2.6Б1.4), (P<.001) and between baseline and four-weeks (2.2Б1.4), (P<.001). There was not a significant difference in step-down test scores between two-week and four-week measurements (P=.18). There was a significant difference in the intrinsic foot muscle grades over the four week period (P<.001). Pairwise comparisons indicated a significant difference between baseline (1.5Б0.7) and two-weeks (2.1Б0.6), (P=.001) and between baseline and four-weeks (2.3Б0.6), (P<.001) for the intrinsic foot muscle test. There was not a significant difference in intrinsic foot muscle test between two-week and four-week measurements (P=.38). Results indicated a significant difference in the LEFS scores over the four weeks (P=.02). A follow-up of pairwise comparisons indicated significant differences between baseline (66.3Б10.1) and four-week LEFS scores (70.5Б7.6), (P=.03) and between the two-week (66.6Б9.3) and four-week (70.5Б7.6) LEFS score, (P=.009). There was not a statistically significant difference between baseline and two-week scores (P=.86). Additionally, there was a significant difference in rearfoot angle over the four week study period (P=.04). Pairwise comparisons indicated significant differences between baseline (6.0КБ2.8 К valgus) and two-weeks (5.0К Б2.2 К valgus), (P=.02), and between baseline and four-weeks (5.0К Б2.6 К valgus), (P=.04). There was not a significant difference between two-week and four-week measurements (P=.87). There were no significant differences for the remaining alignment measurements (all P’s>.05). CONCLUSION: Clinician-observed functional changes and patient-perceived level of function are evident in patients who perform the short foot exercise while undergoing a four-week treatment for a lower extremity injury.
